Alternative Format Textbook Request Form
To request a textbook in alternative format, complete and submit the form below (one book request per form submission).

To complete this form, it will be helpful to have the following information ready:
- your semester course schedule (courses and section numbers)
- a proof of purchase or rental agreement for the title(s) you are requesting
- the course syllabus and assignment schedule

The expected lead time for production of textbooks in alternative format is 15 business days from the date of request. Please review the Alternative Format Request Policy and Procedures page for more information about the Class Materials in Alternative Format accommodation.
